Please have your IRS Letter of Determination and/or EIN accessible along with your most recently completed year's operating budget to supply answers to our questions.

Please read through all these points thoroughly:

The deadline for submission of your Eligibility Form is April 1st or when we have received 50 eligible organizations, whichever comes first.

  • Click on the Apply button below and you will be prompted to login in or create a free Submittable account.
  • Once your account is set up, click CONTINUE in order to access the Eligibility Form (the first step in our application process). 
  • If you need to make changes to the Eligibility Form, we ask that you withdraw your submission and resubmit it before the due date of April 1, 2025. 
  • Once we've reviewed and confirmed your eligibility, we will send you an email with the link to the Application.  Please note our 2025 Application will be distributed to eligible organizations beginning April 1st, 2025. 
  • Once you receive our application, please have prepared for submissions, your organizations current and immediately prior Annual Operating Budget and two (2) years of financial statements -either audited or a Letter of Attestation signed by your Executive Director 
  • The deadline for submission of the Application is May 14 at 5pm
